​​Non‑Attendance Reporting

Providers must submit a Non-Attendance Report within 2 days of the child’s last day of attendance, in accordance with local Child Care Services policy.

When submitting a Non-Attendance Report, the following information must be provided:

*Provider's Details
*Authorized Representative 
*Family Details
*Non‑Attendance Dates 
*Non-Attendance Reasons

 A Non‑Attendance Report is required when:
 A child reaches five (5) consecutive days of non‑attendance, or A child stops attending care during an active authorization period.

*Must be submitted within 2 days of last non-attendance date

All Requests are subject to review and verification.
