Providers may submit a Hour or Holiday Change directly to our staff in order for your Hours or Holidays to be adjusted into Kinder Systems and Amended to the Current Agreement. 

When submitting this request, please ensure the following information is provided:

*Authorized Representative 
* Type of Change (Hours or Holiday)
*Change and Effective Date
*New Provider Operational Hours 
*New Provider Holidays (Total 9 Holiday)

* Changes are not retroactive
*Incomplete requests may delay processing 

By submitting this form, you are requesting a change in the Providers Daily Pay Rate. All Requests are subject to review and verification.

Confirmation

Providers may receive confirmation once the Pay Rate has been updated, or be contacted if additional information is needed.
